OFFICE LOCATION The office of Dennis Kobylarz MD is located in the yellow building at 109 B Main St in Canaan CT. Our entrance is in the rear of the building, down the stairs or down the wheelchair accessible ramp off of the parking lot located behind the building. OFFICE HOURS Regular office hours are: Monday 9:00AM to 5:00PM Tuesday 8:30AM to 5:00PM Thursday 8:30AM to 5:00PM Friday 8:30AM to 4:30PM ACCEPTED INSURANCE PLANS We participate in insurance plans from the following companies [as some companies offer many different plans, please call the office(860-824-0753 ext 2) or email Audrey at: audrey@kobylarzmd.com for further insurance information.
